What's Your End of Life Wish?


Death is a universal experience. Yet, while more than 90 percent of people think that it is important to have conversations about end-of-life care with their loved ones, only less than 30 percent have done so.

For a limited time, we are giving away 500 Heart to Heart Cards - an advanced care planning communication game prepared by Chinese American Coalition for Compassionate Care to help families have meaningful end-of-life conversations.
